ePACT Emergency Form

The two most important benefits of ePACT are requests we have heard time and time again from participants and their families – you will now no longer need to complete a new information form every year, and you will now be connected to a communication network that allows us to text you important program updates!

ePACT Account Setup

Once you receive the invitation to complete your Emergency Form from SEASPAR, doing so is as easy as 1-2-3. Simply follow the steps below to prepare and complete the form. Don’t have an invitation from SEASPAR? Contact us.

Step 1

Prepare to complete the Emergency Form by gathering the following items:

  • Primary parent/guardian/caseworker contact information, including a photo if desired (less than 3MB).
  • Participant contact information, preferably including a photo (less than 3MB).
  • Emergency contact information (individuals other than the primary guardian who may be contacted in an emergency), including photos if desired (less than 3MB).
  • Participant behavior and medical information, including allergies and dietary requirements.
  • A list of all prescription and over-the-counter medications taken by the participant, even if not at program.
  • A copy of the participant’s behavior plan, if applicable.
  • A photo of the participant’s insurance card (required for participants in the EAGLES Adult Day Program; optional for all other participants).
  • A photo of the participant’s photo ID (required for participants who take multi-day trips with SEASPAR; optional for all other participants).
  • A copy of a doctor’s note if the participant has seizures but the guardian does not consent to having SEASPAR call 911 immediately in the event of a seizure.
  • A copy of medication instructions if more information needs to be shared than can be inputted into the Medications section of the form.
  • A copy of any documents regarding special medical needs, if applicable.
  • A copy of any court order or legal agreement related to the participant (i.e., if there is a parent who cannot pick up a participant), if applicable.

Step 2

Using the link in the email sent by SEASPAR, create a free ePACT account. Once logged in, you will see an “Outstanding Organization Request” from SEASPAR to complete your participant’s information. Click “Complete Request” to complete the Emergency Form in the following sections:

  • Contact Info: This section is for the primary parent/guardian/caseworker’s contact information. (Additional legal guardians can be added in a later step.) The mobile phone numbers entered on this page will be used for text message communication for program updates, if you choose to opt in. Adding a photo of the parent/guardian/caseworker is optional.
  • Dependent’s Details: This section is for the participant’s contact information. Adding a photo of the participant is not required but strongly encouraged for safety.
  • Family Info: Here, you can add additional legal guardians. We discourage entering sibling information unless that sibling is a SEASPAR participant or is an emergency contact. When entered, siblings will automatically be added as emergency contacts.
  • Emergency Contacts: This section allows you to add additional emergency contacts OTHER than legal guardians or siblings you’ve entered, who are automatically emergency contacts. In the Pick-up List, please list any carpool friends if applicable, but do not reenter guardians or emergency contacts. You may also identify individuals who are not allowed to pick up the participant, if applicable.
  • Additional Information: Here, you will enter information about the participant regarding behavior, transportation, etc.
  • Medical Info: This section is for information regarding the participant’s medical conditions.
  • Allergies & Dietary Requirements: Here, you will enter any food, environmental, or medication allergies the participant has, as well as any dietary needs.
  • Medications and Treatments: In this section, you will list all of the prescription and over-the-counter medications the participant takes, even if not taken at program.
  • Transfer Plan: If the participant uses a wheelchair, this section requests more information.
  • Toileting Plan: If the participant requires any assistance with toileting, this section requests more information.
  • Seizure Plan: If the participant experiences seizures, this section requests more information.
  • Shared Files: This is the place to upload the participant’s behavior plan, insurance card, etc.
  • Complete Info: Here, once all of the required information has been provided, you will certify that the information is accurate and complete to the best of your knowledge, then submit the form. If you receive a message that the information is not yet complete, that means that a required question has been missed. A red triangle next to the page title in the left navigation will indicate what section needs more information.

Step 3

Celebrate! You have finished the one-time Emergency Form completion. Be sure to update the form throughout the year if any information changes. At the end of the year, we will simply ask you to review and recertify the form – you won’t need to fill it out again!

Troubleshooting

Having trouble with your form? First, check to make sure that all required questions are answered. This is the most common problem!

Also be sure that ePACT is only running in one web browser tab or window. If the form is open in multiple tabs, it will time out. The form will also time out after 30 minutes of inactivity.

Use the “Continue” button to save your work. You can leave and reenter the form as many times as you need.

Please note that SEASPAR staff cannot edit the information you enter into ePACT. You maintain full control over the information you provide, and all new information or changes must be entered by you.

Reset Password

For assistance with resetting your password, please contact ePACT at help@epactnetwork.com or call 855.773.7228 ext. 1. Please note that SEASPAR staff cannot access or reset your account password.

Update Account Email

  1. Log in to ePACT with your old email address and password.
  2. Click the menu button in the top right corner.
  3. Select Account Settings.
  4. Under Change Email, type in your new email and existing password.
  5. Click Submit when complete.

Update Emergency Form Information

  1. Log in to ePACT.
  2. Click My ePACT at the top.
  3. Under My Household, click the participant’s name. In the box that pops up to the right, click SEASPAR Special Parks and Recreation. This will display the submitted information.
  4. Scroll down to the section you’d like to update, and to the right you’ll see an Edit button with a pencil. Click that.
  5. Update information as needed.
  6. Click Save when complete.
